Background: Moderate head injury is a traumatic condition with a Glasgow Coma Scale (GCS) score of 9–12 that may cause hemodynamic instability and increase the risk of secondary brain injury. One non-pharmacological intervention is the semi-Fowler/head-up 30° position.

Purpose: To determine the effectiveness of the semi-Fowler position in maintaining hemodynamic stability in patients with moderate head injury in the Emergency Department of PKU Muhammadiyah Hospital Yogyakarta.

Method: This case study involved two patients with moderate head injury. Data were collected through observation and documentation. Patients were placed in a semi-Fowler/head-up 30° position. Hemodynamic parameters, including blood pressure, pulse rate, respiratory rate, body temperature, and oxygen saturation, were observed before and after the intervention. Data were analyzed descriptively and presented graphically using Microsoft Excel. The study was conducted from December 29 to 31, 2025.

Results: The results showed improved hemodynamic stability after the intervention, indicated by decreased blood pressure, pulse rate, and respiratory rate, as well as increased oxygen saturation in both patients.

Conclusion: The semi-Fowler/head-up 30° position helped maintain hemodynamic stability in patients with moderate head injury by improving cerebral perfusion, enhancing oxygenation, and reducing intracranial pressure.
