Background: Hemorrhagic stroke is a neurological emergency that can increase intracranial pressure and impair cerebral perfusion. Patients with decreased consciousness are at risk of airway obstruction and hypoxia.

Purpose: To describe airway management, a 30° head-up position, and cerebral perfusion monitoring in a patient with hemorrhagic stroke and decreased consciousness in the Emergency Department.

Method: This case report involved a 60-year-old patient. Data were obtained from nursing assessment and medical records. Interventions included maintaining airway patency, oxygen administration, head elevation to 30°, and periodic monitoring of the Glasgow Coma Scale (GCS), vital signs, and oxygen saturation.

Results: The patient initially had a GCS score of 10 (E3V3M4), blood pressure of 180/110 mmHg, respiratory rate of 28 breaths/minute, and oxygen saturation of 90%. After approximately two hours of intervention, oxygen saturation increased to 96%, respiratory rate decreased to 22 breaths/minute, and blood pressure decreased to 160/100 mmHg. The GCS score remained 10 without further deterioration.

Conclusion: Airway management, a 30° head-up position, and cerebral perfusion monitoring were followed by improved oxygenation, a lower respiratory rate, and maintained neurological status.
