Background: Acute Respiratory Infection (ARI) remains a common infectious disease and a public health problem in many countries, including Indonesia. ARI can impair Eustachian tube function, causing fluid accumulation in the middle ear and increasing the risk of Acute Otitis Media (AOM).

Purpose: To determine the association between a history of Acute Respiratory Infection and the occurrence of Acute Otitis Media.

Method: This analytical observational study used a cross-sectional design and secondary data from patient medical records. A total of 150 medical records that met the inclusion and exclusion criteria were selected using purposive sampling. Data were analyzed using the Chi-square test, and the strength of association was assessed using the Odds Ratio (OR) with a 95% Confidence Interval (CI).

Results: Among 150 medical records, 118 (78.7%) had a history of ARI and 120 (80.0%) were diagnosed with AOM. Statistical analysis showed a significant association between a history of ARI and the occurrence of AOM (p = 0.041). Patients with a history of ARI had 2.73 times the odds of experiencing AOM compared with patients without a history of ARI (OR = 2.73; 95% CI = 1.13–6.58).

Conclusion: A history of Acute Respiratory Infection was significantly associated with the occurrence of Acute Otitis Media. Early detection and appropriate management of ARI may help reduce the risk of AOM and its complications.
