Background: Smoking may affect blood glucose regulation. Harmful substances in cigarette smoke, including carbon monoxide, tar, and nicotine, may impair glucose metabolism and contribute to insulin resistance. Passive smokers may also experience health risks from inhaling cigarette smoke.

Purpose: To compare fasting blood glucose levels between active and passive smokers working in the hospitality sector.

Method: This observational cross-sectional study included 32 respondents: 16 active smokers and 16 passive smokers, selected through purposive sampling. Fasting blood glucose levels were measured using the glucose oxidase–para-aminophenazone (GOD-PAP) method with a Chemistry Analyzer (TMS 30i). Data were analyzed using the Shapiro–Wilk normality test and an independent-samples t-test.

Results: High fasting blood glucose levels were found in 11 active smokers (68.8%), whereas 12 passive smokers (75.0%) had normal levels. The independent-samples t-test yielded t = 4.699 and p = 0.000 (p < 0.05), indicating a significant difference between the groups.

Conclusion: A significant difference in fasting blood glucose levels was found between active and passive smokers among hospitality workers in Yogyakarta City. Levels tended to be higher among active smokers.
