Background: Recovery following general anesthesia is commonly assessed using the Aldrete Score in the Post-Anesthesia Care Unit (PACU). However, routine assessment of delirium remains relatively uncommon. Delirium is an acute disturbance of brain function frequently observed in postoperative patients, particularly older adults, and is thought to be associated with the patient's recovery status following general anesthesia.

Purpose: To determine the relationship between Aldrete scores and the incidence of delirium among post-anesthesia patients in the PACU at PKU Muhammadiyah Yogyakarta Hospital.

Method: This study employed a quantitative observational analytic design with a cross-sectional approach. A total of 48 respondents were selected using purposive sampling. Data were collected using the Aldrete Score observation sheet and the 4AT (4 A's Test) instrument for delirium assessment. Data were analyzed using the Spearman Rank correlation test.

Results: The Spearman Rank correlation test showed a p-value of 0.010 (p < 0.05) with a correlation coefficient of r = -0.368. These findings indicate a statistically significant relationship between Aldrete scores and the incidence of delirium among post-general anesthesia patients in the PACU. The correlation was weak and negative, suggesting that higher Aldrete scores were associated with a lower likelihood of delirium.

Conclusion: There is a significant relationship between Aldrete scores and the incidence of delirium among post-general anesthesia patients in the PACU at PKU Muhammadiyah Yogyakarta Hospital. Patients with higher Aldrete scores tend to have a lower risk of experiencing delirium.
